We report on a 67-year-old female patient who presented in July 2005 with sudden onset of pain in the right upper abdomen. The patient had undergone cholecystectomy in 1987. Because of recurrrent complaints in the right upper abdomen, a pigtail stent was placed into the common hepatic duct in 2001. When the patients presented now, the laboratory tests including liver enzymes were within normal ranges. Endoscopic retrograde cholangiogi aphy, however, revealed a remaining 10-French, impacted double pigtail endoprothesis that was obstructed by sludge as well as multiple giant bile duct stones of 20 to 30 mm in size. Serous Macular Detachment Associated with Dome-Shaped Macula and Tilted Disc

Introduction: An entirely new type of staphyloma has been recently described as dome-shaped macula (DSM). It is characterized by an abnormal convex macular contour within the concavity of a posterior staphyloma. We found DSM associated with serous macular detachment (SMD) and tilted disc in two consecutive cases. Case Reports: Case 1: A 37-year-old female presented to our department because of sudden onset blurred vision in her right eye (OD). The best-corrected visual acuity (BCVA) was 0.5 in both eyes. Children complaints dizziness merit meticulous evaluation to differentially diagnose a vestibular disease. A syndrome mimicking certain classic signs and symptoms of adult vestibular disorders may be presents in children, such as benign paroxysmal positional vertigo, usually associated with aging. Benign paroxysmal vertigo, in which migraine is a manifestation, with sudden onset of dizziness is a rare peripheral vestibular disorder that is commonly ignored or misdiagnosed. This review covers the development of the diagnosis, evaluation and treatment approaches of vertigo of childhood, a valid support for physician that approach dizzy children (Ref. 25). The Proton-Neutron Interaction in Neutron-Rich A ≃ 100 Nuclei

The possible factors responsible for the sudden onset of nuclear deformation in neutron-rich A≅100 nuclei are reviewed from the systematics of subshell occupation numbers for neutron and proton orbitals. The agreement between our results from a Nilsson model with existing selfconsistent HFB calculations strengthens the role of a cumulative neutron-proton interaction, but does not substantiate the conjecture concerning a selective involvement of the πg9/2-νg7/2 spin-orbit-partner orbitals in producing deformation as das suggested in earlier work.

A major challenge lies in balancing the complexity and uncertainty of the problem with time and resource constraints. To address this problem, we propose a location–allocation model that divides the topography of affected areas into multiple layers; considers constrained number and capacity of facilities and fleets; and allows decision-makers to explore trade-offs between response time and logistics costs.
